When your goals become more comfort focused rather than curative care.
When you have been told by a physician that you have a serious illness, and that you may have 6 months or less to live.

Hospice is a choice. Patients can leave at any time for any reason. Patients can also come back to hospice at any time, as long as they meet the eligibility criteria.
